Reablement is a word that gets used in a lot of different ways, so it is worth being clear about what we mean by it at Axon Neuro. For us, reablement is short, focused support that helps a person do more of the things that matter to them, in the way that suits their life.
It is not about doing tasks for someone. It is about working alongside them, at their pace, so that skills and confidence can be rebuilt after a brain injury, a stroke, or while living with a neurological condition.
Starting with the person, not the diagnosis
Two people with the same diagnosis can have very different days, goals and home lives. So we start by listening. What does a good week look like for you? What feels harder than it used to? What would you like to be able to manage on your own again?
From there we agree a small number of goals together. These might be practical, such as preparing a meal or managing a weekly routine. They might be about confidence, such as travelling to a familiar place or returning to a hobby. The goals belong to the person, not to us.
How the support is organised
A reablement plan with us usually involves:
- A clear set of goals, agreed with the person and anyone they want involved
- A coordinated team, so therapists and support staff are working towards the same things
- Regular reviews, so the plan can change as someone makes progress
- An honest conversation about what is realistic and what might take longer
We keep instructions clear and the pace comfortable, because cognitive fatigue is real and progress is rarely a straight line.
Working with families and funders
Many of the people we support are referred through NHS Continuing Healthcare, local authorities or case managers, and some arrange support privately. Whatever the route, we aim to keep communication open and plain, so everyone involved understands the plan and the part they play in it.
Why we are writing this
This is the first article on our blog. Over time we want to share practical, person-centred notes on reablement and rebuilding independence, written in plain language. If there is something you would find useful to read about, we would genuinely like to hear it.
