Axon Neuro is a neuro-rehabilitation reablement service supporting adults across England. We bring together a connected multidisciplinary team to deliver personalised support that helps people regain confidence, skills, and independence in everyday life.
Our approach
Reablement is about doing things with people, not for them. We start by understanding each person, what matters to them, the routines they want to rebuild, and the goals they are working towards. From there we agree a plan and a pace that feels right.
Our support is coordinated across a multidisciplinary team so the right expertise is in place at the right time. We review progress regularly and adapt the plan as confidence and independence grow, keeping families and referrers informed throughout.
